At Divers Zorg you will find a broad range of care and support. Every form is tailored to your personal situation, with individual attention as the starting point.

Individual support

Sometimes you need support in giving your day some structure, in becoming more independent or in building up social contacts. Our support workers work with you on goals you can reach — at your own pace and suited to your situation.

Supported living

With supported living you live independently, while we help you with the everyday things that are sometimes difficult. That way you grow towards more independence, with a safety net in the background.

Sheltered housing

A place of your own where you find rest and safety. In a protected living environment, specially trained support workers are there for you day and night. That gives you the structure and the care you need, in a warm and safe atmosphere.

Case management

Your case manager keeps an overview of all the care and support you receive. He or she coordinates the organisations involved, watches over the agreements and is your regular contact person — so that you do not have to worry about the organisation around it.

Day activities

We support you in spending your day in a pleasant and fitting way. Together we look for activities that suit you and that give meaning — whether that is meeting others, developing yourself or simply a useful way to fill the day.

Personal care

When you cannot fully look after yourself, for a short time or for longer, we offer help with personal care. Think of support with washing, getting dressed and other daily tasks — always with respect for your dignity.

Home help

When the housework is too much to manage on your own, we are glad to help. Our staff support you with the daily household tasks, so that you can keep living in a clean and pleasant home.

Group support

In a group we do activities together. Contact with others, structure and a feeling of belonging are central. Our support workers create a safe and pleasant atmosphere in which everyone comes into their own.

With us you arrange the care with a pgb (a personal budget you use to buy your own care), which you apply for under the Wmo (the Dutch Social Support Act, arranged by your municipality) or the Wlz (the Dutch Long-Term Care Act). Read more about how care is paid for.
